Twins HomeCare is proud to offer home and community-based services in accordance with Minnesota's 245D waiver programs, serving individuals in the Twin Cities area. Our services focus on empowering individuals to live independently in their homes and participate fully in the community. We specialize in providing Individualized Home Supports (IHS), offering support and training to adults aged 18 and above in their own living units or in the community.

Our support services include cueing, skill maintenance, guidance, instruction, assistance with activities of daily living, coordination of community living activities, and direct supervision. Our training services are tailored to each person’s specific needs and encompass skill-building and instructional services designed to acquire, retain, and improve the individual's experience living in the community. We are dedicated to helping each person achieve greater independence and a higher quality of life through personalized support and training.

Individualized Home Support (IHS)

Services for people who live in their own home or family home that provide support and/or training in the community living service categories listed in the covered services section.

There are three types of IHS:

  • Without training.

  • With training.

  • With family training.

All types of IHS can be provided in the person’s own home, family’s home, or in community spaces used by the general public, and either in person or remotely.

Crisis Respite

Twins HomeCare provides short-term care for individuals needing intensive support to help them progress, stabilize, and transition. Our crisis respite services act as a steppingstone back to the community, facilitating each individual's return to a more integrated and independent setting. We are dedicated to offering a safe and supportive environment that promotes healing and growth during times of crisis.

Adaptive Skills

We focus on implementing positive support strategies that encourage self-sufficiency and foster problem-solving skills. Our approach integrates crisis prevention techniques and tailored training to enhance positive behaviors, resulting in the reduction or elimination of challenging behaviors. By emphasizing sensory and motor development, we help individuals acquire essential functional skills, empowering them to navigate daily challenges with confidence and independence.

Integrated Community Support (ICS)

we provide efficient Integrated Community Support (ICS) services, offering support and training to adults aged 18 and above in their own homes or in the community. Our services empower individuals to live independently and participate fully in community life.

Our support includes cueing, skill maintenance, guidance, instruction, assistance with daily living activities, coordination of community activities, and direct supervision. We tailor our training to each person's specific needs, focusing on skill-building and instructional support to help individuals improve their experience living in the community. Our goal is to enhance each person's independence and quality of life by providing the necessary tools and support for successful community living.

Household Management

At Twins HomeCare, we provide cueing, guidance, supervision, training, and instructional support to assist individuals in completing routine household care and maintenance tasks. We emphasize the development of household safety knowledge and skills to ensure a secure and safe living environment. Our services include tenancy support and advocacy, helping clients navigate housing matters effectively. We offer training, assistance, and guidance in budgeting and money management, cooking, meal planning, and nutrition. Additionally, we promote healthy lifestyle skills and practices, support with household chores—including minor household maintenance activities—and assist with personal-needs purchasing. Our comprehensive approach is designed to empower individuals to manage their households independently and confidently.

We work closely with individuals to coordinate comprehensive health care services—including physical, mental, and chemical support—along with meaningful activities and social services. Our collaboration involves arranging health care appointments, social services, meetings, and engaging activities. Through our training and support, we empower individuals to complete self-care tasks, including activities of daily living (ADLs), and provide health services support to foster resilience. We are dedicated to helping individuals activate and build resiliency factors, designing and achieving personalized strategies to meet their health, safety, and wellness goals.
